By bullies do you mean people in charge or other CSO's?

For things to change internally there needs to be changes made by the poers that be (ie the government). Yes I agree major changes need to be made and they needed to be made yesterday! This issue is as well, where do you start? It's going to be such a monumental task but definately one that is needed.

Children shift from different carers for a number of reasons. I believe that continuity is important for children no matter in who's care they are. Sometimes they are moved for the wrong reasons and sometimes they are moved because of situations that are out of Child Safety's control. Relative carers are so important to children. I say they should be the first option every time, permitting that they are a safe option of course.
